Using the WordPress Uploader in Plugin or Theme

I have integrated this trick to my theme options https://www.webmaster-source.com/2010/01/08/using-the-wordpress-uploader-in-your-plugin-or-theme/ . Now my question is, how do I show the image in the theme template? I am not able to understand how to get the URL of uploaded picture. Thanks for help.
