Using MaxButtons to call up Ninja Forms Modal Pop Up

Hi,
I am looking at using MaxButtons to call up a Ninja form through the modal pop-ip plugin.
MaxButtons is asking for Extra Classes, or Custom Rel tags.
What information from Ninja Forms would I enter in to these?
