Using a Quform as the GWA form

I would like to use Quform for people to subscribe to a newsletter as it looks better is more useful than the one with GWA.
What I want to do is to allow people to choose the form they want to subscribe to via a tick-box or drop-down on a contact form. How would I go about achieving this? or what is the short-code to put in the form?
