Users being blocked but not added to list

    We use this plugin on a members site and for some reason members keep getting blocked even when just requesting a password change, but they don’t show up in our logs, it’s like something is triggering the script without actually entering a username or password.

    Anyone experienced this or have an idea of how we can diagnose why it happens?
