Username already exists error

Using this plugin for SSO/SAML authentication, and over the past 3 days we’ve had 3 people email for support because they get a “Username already exists” error when attempting to authenticate and access the site. We have “Create user if not exists” set to true and use Microsoft Online as our SSO service. Anyone else seeing this and have any ideas to the root cause?
I should mention that manually deleting the user from the WP Users clears up the problem.
