User table submission

  • Hi All,

    I’m building a weight loss program and I’m really interested in creating a form for users to submit their weekly measurements for 9 weeks so that the data is displayed in a table for them with a total at the bottom for each week. And I would also liked the table to be emailed to me each time they add a weeks measurement in.

    I would like it to look something like this:

    If anyone knows of a plugin that could be used to create this or if you know how to write this kind of a plugin it would be very much appreciated.
