User Role Editor Permissions change

Hello,
Recently my multi-site has been having issues regarding permissions.
For some reason users in the dashboard won’t be visible even though they exist. On top of that admin’s can’t add other admins or change permissions of lower user roles.
This seems to happen randomly and effects all of my sub-sites. Any ideas as to what could cause this to occur?
