User Role changed not sending

Hello,
I think this is a fantastic add-in
I have it setup and it is working great and sending most notifications but it is not sending role changes notifications.
I have Members and Post SMTP installed as well. When I look at the Post SMTP log I see new user, and Password Change which are the only notifications I have setup but no role change notifications. So I don’t think it is an email issue. any idea on what might be wrong or how to troubleshoot the issue?
