User Registration Form Unable to Create Account

    Hi,
    I just created a User Registration form using this Plugin, the form gets submitted successfully BUT the user account is not being created.

    I have tried it with all 3 options i.e.,
    1. Default, 2. Email Activation and 3. Manual Approval

    BUT none of them have worked for me. I will really appreciate, any assistance to make it happen for me.
