    I hope you’re fine today!

    This definitely looks to be conflict of some sorts.

    Would you please:

    – go to that “user registration” page of the form in Chrome browser
    – then open browser console (CTRL+SHIFT+J on PC/Cmd+Option+J on Mac)
    – reload the page and see if it shows any errors there?

    If it does, let us know what are those errors.

    If there is no errors there, then, I’m afraid, a full conflict test will be necessary:

    – temporarily disable all other plugins (except Formiantor) and switch theme to Twenty Twenty-Three
    – confirm that issue is not happening
    – start enabling your plugins back one-by-one after each one checking for the issue
    – at the end enable theme and check again

    At some point issue would get back so the last enabled plugin (or the theme) would be the cause of the conflict and knowing about it we could then look closer to it to check why it’s happening and how to fix it.
