User generated Doodle-like form

Hi

I’m looking for a plugin that would allow users to fill forms, but in a way that their answers would be visible on the page and editable afterwards. In a similar manner as Doodle(dotcom) works, but with any type of columns. If the plugin allows file upload to the form, it is a huge plus.
