user details required

Hi!

I set require the users name, telephone and email (in options), aftet that i save options and I click preview. It,s okay, but when i log out the wordpress and i see the quiz i send the quiz without any information. What will be the problem? i hope u understand my letter/my problem. I,m waiting for your answers. it,s so important!!! Thanks! ??
