User account creation problem

    We had a weird issue earlier today when a site visitor submitted our registration form that also creates a user account. She was in the Inbox, but she was not in RM’s User Manager, nor in WordPress’ Users.

    We immediately tested form by submitting it ourselves and it worked as expected – a new user was created & automatically activated, and was visible in the Inbox, User Manager, & WordPress Users.

    Any idea why this might have happened? A server hiccup or something?
