Submit Post Popup

1st of all, thanks for this amazing plugin.
I need urgently help, Please check

1st – I have custom post type and it have custom fields, I want to show only those custom fields in popup with a submit form where in one field will be post title, another field will be one taxonomy and next one also will have taxonomy. In one field will be person’s name and in one will be email if they are logged in otherwise it will be empty. So indirectly I will have a submit post popup where I want to show these fields.

2nd – This time I am seeing only blank popup
