Upload File Field not succesfull

i created a form kuesioner and add an upload file field in there. but somehow, when someone submit the entries sometimes the form not capture the attached file (the views like blank picture and the size only 0 b) and sometimes the form capture the attached file. what I need to do for solve this problem?
