Upload errors

I have been having issues with easy contact form from people trying to upload images. simple jpg files. one person notified me she was using a apple another said she was just trying to upload on windows 8 and just uploading a .jpg file. file was less than 1mb. I have been trying to contact easy contact forms for about 3 months now. about six emails. I am hoping somebody else has come across this issue and might know a fix.
