Upload button grayed out / Replace disabled

I am unable to replace the media from using this plugin.

When I choose Media Replace from Media Library, I notice that the red ‘Upload’ button is “grayed out” and unclickable, even after choosing a new media file and changing other options.

I disabled all plugins except Enable Media Replace but the problem persists.

Please help me resolve this issue.
