i changed mine to List View from grid. then added extra fields where it says

Choose field(s) to display in reveal section

works perfect for me.

for your problem might neeed to create a new custom field with that name for

Choose filter(s) meta to enable

That seems to help the first issue 90%. (I really liked how the search by fields displayed first; with the search button after.) I will try to center the search a bit more and the search button is pushed lower for some reason. I will play with come CSS.

^ there are arrows under each providers name, which is the second issue I am facing.

Since the update on the UM plugin some arrows have randomly disappeared. (They are displayed after a search though, just not right at page load.)

If the profile has no extra information to show, the arrow-down will not show.

But every profile has the extra information. The extra information is only the “Location” the physician works at.

UPDATE: I can see that only certain locations are in the dropdown. I am not sure why half of them are missing?
