Upgrade to 1.4.3 broke plugin

    Help-
    After the upgrade to 1.4.3 none of my conditional fields work properly – the date fields are gone completely from the front end of the form, none of the conditional fields entered info is captured and emailed to the specified addresses. Urgent help needed thank you!
