Updated to newest version and form no longer displays

Hello,

I recently updated the plugin to the newest version, and now the form does not appear on my page. All that’s listed is this shortcode:

[si-contact-form form=’1′]

I also cannot find the plugin on my dashboard to view and edit my forms as I had done before I updated. Help is much appreciated.
