Unwanted Black and White Bars below “Submit” Button

I’ve created a new Subscription popup and need some guidance to get it right…
1. It’s too tall and creates a vertical scroll bar. How can I reduce the height of the popup window?
2. There are unwanted black and white bars below the “Submit” button. You can see what I mean at this image: Link to Screen Capture
3. Am I correct that it will automatically connect with my existing Popup Subscribers list?
