Unknown Error Occurred E01 on Mobile Devices

I noticed a problem with customers trying to submit a review on a mobile device (tested ipad and iphone) and I received an error message on both that states “An unknown error has occurred E01” This error doesn’t allow the user to submit a review on a mobile device. I tried to test this by posting a review on a desktop and it worked just fine. I have updated my theme and wordpress to the latest version to see if it would solve the problem but this didn’t solve the issue. I am running 3.1.3 on the Customer Reviews plugin as well. Anyone else experience this? Website name is OriginalShaveCompany dot com.
