Unique Key Constraints errors, WP 3.5.1

I updated a site to WP 3.5.1 and am now getting frequent errors “WordPress database error 23000”. Example copied below. We are running Windows Server 2008R2 and SQL 2012. Using DB Abstraction 1.14 with corrections listed in this thread:

Same error cropping up for various sites all running different themes, plugins. Any suggestions on where to look would be helpful. Thanks!
