We replied to the queue order and hence the delay in getting back. I checked the form setting and there isn’t anything specific within Forminator side that’s causing the issue. The email in the form in general works fine when tested on my test site.
What you have pointed seems to be more specific to the WP side or Email Hosting configuration which is something you’ll have to check with your hosting provider.
To double check would recommend you to install Check & Log email which helps with sending test emails from your WP, so that you could test and see on when it fails:

If the issue is still the same with the above plugin it would mean that the issue isn’t related to Forminator.
You can also enable the Logging feature in the above email to track errors picked when sent to @crossfithollywood.com which could be escalated to your hosting provider.
I installed an SMTP plugin, but was having issues with that as well as it was unable to send a test email to?[email protected].
When testing please do check whether the “From email” address is the same as your admin email address. There can be restrictions from the hosting side if they are different. Did you check the Email logs for any errors regarding this?
As far as the password reset subject, do you mean an ID with @crossfithollywood.com? that is the email address ending we are having issues with.
The above actions were suggested to check whether emails work in general or not. You can use the Check & Log email plugin to test instead of the above action.
If i change the entire website’s Administration email address, might that help with anything? I am hesitant to do that as I don’t want to create more problems.
It would be better to 1st check the above aspects and if you still have issues with sending emails through the Check & Log email even with SMTP for @crossfithollywood.com, then it would mean either any existing plugins are causing a conflict or more of an issue within the hosting side.
You can check and see whether you could replicate the same issue with all the plugins disabled except SMTP, if it’s still the same then I would recommend you to get in touch with your hosting provider.
Please do note that the issues mentioned isn’t related to Forminator, so what we could assist is limited regarding the issues with SMTP or emails as it is more related to the WP site or the Email host.
