Unable to log into wp-login.php

Did you enable email only login?

Yes.

When I go my login page at:

Whenever I enter either my username or email address It is telling me it is incorrect.

I have installed the Theme My Log In plug in on a twenty eleven theme.

It made log in, lost password, reset password and register pages.

The register page has a form that works, which is great.

The log in page has a title of LOG IN, and the links Dashboard, profile and logout.

Dashboard sends to me to the site Dashboard
Profile sends me to a form
And log out sends me to a page that says This is somewhat embarrassing on a tabbed page that says that the page is not found.

I am new to all of this and have obviously missed something. Can someone help me?
