We have checked your screenshot and site too.
Form is visible in popup.
But the popup is different from what you are referring in screenshot.
Can you please check this once again from your side?
Or let us know what settings are not reflecting?

Hi Sandhya,

The form seems to be working now. However, I have set the Form Actions to send it to a PDF download after the user fills in the form. The form doesn’t redirect the user to the PDF rather just display Thank You message.

Please guide me how to resolve this.
