Unable to Create New Vendor (Front End)

WooCommerce – 3.1.2
Wordpress – 4.8.1
PHP- 5.6
Current Theme – Peak Theme from themify. Same issue with Storefront theme

When I try a add vendor (from Order screen)

Your account is not setup as a vendor.

[check box] Apply to become a vendor?

[check box] I have read and accepted the terms and conditions

When I press submit button,nothing happens, I get back to the same screen.

(I created user with vendor role from back end. That user can log in etc)
