unable to create an app in db_toolkit

When I click on create app in the db_tookit app builder it puts a form at the top of the page instead of bring up a pop up window with the form in it. this would be ok but there are no buttons to exicute the building of the app. I am running wordpress 3.6.1 on a windows 2008 server with IIS7.
Any help would be appreciated?
