ultimate member registration form not working

before the form was working fine but when i added a few fields (i.e. phone number , email, mobile number) the form stopped working and as in the screen shot the registration form was not showing. Any help would be appreciated, and please help i’m stuck on this thank youThe page I need help with: [log in to see the link]
