    @garyifpg – Hopefully this clarifies the issue. The form doesn’t appear to be one of ours and even then we tend to leave the visual appearance to your theme (as it should be styling those elements with defaults that are appropriate), but often isn’t the case or requires slight modifications.

    In this case if the form is from another plugin, we have nothing to do with how it looks if that makes sense.

    Hope that helps, but might be best to check with the theme first and then that other plugin.
