Two column Calculators?

I’m trying to create a form that will calculate two separate variables, then summarize the end values into one. Something like –

Input 1 _________ Input 2
Input 1a ________ Input 2a
Result 1_________ Result 2

_____Total Results
____Result1 + Result2

Currently all templates’ are only allowing for me to create something like the Input 1 List, but I would really prefer to have a 2-column Calculator.

Any assistance would be great. Thank you!
