Checkboxes you are seeing are from groups that you have in your Mailchimp list.

When you go to your opt-in and select Mailchimp group you will see additional explanation of what can be done with those groups and which ones you want to show.
Or you can go to your group settings directly in Mailchimp directly and select “Don’t show these groups on my signup form” option.
