Trying to find a lead generation/referral plugin

  • I’m interested in moving to a site over to WordPress, but have a fairly complicated form that either needs to be replaced, or be migrated over. The form works by allowing the visitor to enter their postal code; then displays 3 of our closest contractors, and emails the visitor’s information to the contractors. Allowing the contractors to contact the visitor to find out specifics regarding their project.

    My searches have come up empty-handed. Are there any plug-ins available that do something similar or is this something that would need to be custom built?
