Trying to create online trail log

I’m trying to create a way for contributors to easily create an online trail log or trail review (for hiking and biking trails). I want a submission page that prompts them to complete certain fields with information, then have that be formatted for the post, as opposed to a free-form type of post. (ie: Trail Name: _____, Location : _____). Any ideas? How can I modify the posting page to accommodate this? I tried to adapt other things (a book review plug-in modified would work)….however most of the ones I’ve found do not work on the current WP. Thank you, very kindly, in advance.
