Trouble with adding the contact form

This chained quiz plugin is too good to be missed out. But there is an issue. We are unable to include the contact form in the quiz. It is possible to add media which is shown in the sent mail to the participant. We can also add the shortcodes from the other contact forms which is also seen only in the sent mail. But we need the participants data before they attempt the quiz / survey. We could create a form through the link but once the ADD THE FORM TO MY POST is clicked, it goes nowhere. Its stuck. Please help. Thank you.
