Triger the form from other button

What im trying to do is triger the fixed form using a button on the page.

The idea is this.
A report button a custom post type.When user click on it the fixed form from the right bottom corner opens and the user write the reason of the report.

In the email now because we dont want to ask the post url or post name we have to include where the report button pressed and include that url in the email.

Right now i use contact form 7 for that but kanzu i think can do better the work ??
