Tree structure not retained

I am new to using this plugin so am trying to find my way around.
First off, it doesn’t do quite what I expected. I thought it would automatically create a hierarchical tree structure based on the embedded links in the pages. In fact, it looks as if you have to create the tree manually from a list of pages that initially is simply ordered alphabetically.
I can move pages around and nest them but when I navigate away from the editing page, the new structure is lost. There does not seem to be an update button (say) to ensure your changes are kept. I feel sure there must be a way to retain structure. Can anyone tell me how?
