top banner lost in other pages, help please

1. I have a top banner here, in main page:

2. it is visible in single pages:

but it is lost in CATEGORY page:
https://www.aligurtuna.com/category/bebek-cocuk-hastaliklari

it is lost in TAG page:
https://www.aligurtuna.com/tag/saglik

it is lost in my ARCHIVE page:
https://www.aligurtuna.com/date/2011/02

i use WP 3.05 with blucorp theme.
there is no category.php or archive.php or tag.php in my theme folder.
i put index.php here:

please help me.
