Tooltip in a form

Hey, guys! I’ve been trying to add a tooltip to my form for a while now unsuccessfully.
The exact type of tooltip I’d like to add to my form is this one in the picture:

It would be ideal that the user could hover on the question mark icon and the tooltip would popup like in the photo, but if I could find a way to get a tooltip like this but only shows up after a click it’s alright as well. Thanks!
