TOC of complete website

Hi all,
I need to create a table of contents with h2 headings of ALL pages and ALL posts in one table to implement in a single page called ‘Sitemap’.

1. Page 1
1.1 h2-Heading 1
1.2 h2-Heading 2
1.3 h2-Heading 3
1.4 h2-Heading 4
2. Page 2
2.1 h2-Heading 1
2.2 h2-Heading 2
2.3 h2-Heading 3
2.4 h2-Heading 4

as well as

1. Post 1
1.1 h2-Heading 1
1.2 h2-Heading 2
1.3 h2-Heading 3
1.4 h2-Heading 4
2. Post 2
2.1 h2-Heading 1
2.2 h2-Heading 2
2.3 h2-Heading 3
2.4 h2-Heading 4

TOC PlugIns are doing well but only sidewide. I need a solution for the complete website. Any idea guys?
