Title Text

I am using a site called Raventools to help me find out some SEO problems with my websites. They seem to think that all of my images have no title texts. I have gone into every page and added them, as well as the media library. I still am getting told their are no title texts on some of my images. Side note, some of the images that appear are written in code only, so really they have no image but title text is listed correctly in the code. What am I doing wrong? Also it happens a lot on my Widget images which from what I can tell, are all set up correctly.
