Title showing in menu bar

I’ve noticed that anytime I click on a specific tag in the list I get a strange menu item appearing. Seems that somehow the title of a post will show up before the Home menu position. It’s the same post every time. I can’t figure out how it is being picked up. Anyone else have this problem?
