Title Repeats On Every Page

It was happening to me as well when I was using WP SEO. you should delete the site title and description from the SEO field.

I just looked and the problem still persists for some reason. :/

I’ve also deactivated and removed the plugin only to have the issue persist.

It’s been re-added again also.

I’m still what can be considered a newb so maybe I’m missing something very obvious. Here’s the header.php:
