Title issue

This is my site: getpnrstatus.co.in

I don’t wanna continue “Blog title” after “page title”. I have did all the things which i can but i can’t get rid of this problem.

All in Seo Pack Settings:

Use Original Title: Enabled
Rewrite Titles: Enabled
Page Title Format: %page_title%

Site:

Header.php :

<title><?php
if ( is_home() || is_front_page() ) { bloginfo(‘name’); }
elseif (is_page(”) ) { single_post_title();}
elseif (is_single(”) ) { single_post_title();}
else wp_title(”); ?></title>

Page.php :

<h1 id=”page”><?php the_title(); ?></h1>

Please help me out, i am fed up of this problem.
