Timestamp Extra Form?

On the post page, is there anyway I can add another form, right after the year? And also, how do I append this to the timestamp on the actual post?

For clarification, I’m talking about the publish box. Inside, when you click edit(next to the publish time) you are able to change the time, but I want an extra form next to the year, and I also want to display this.
