Check into Lester Chan’s suite of plugins: WP-Ratings should be the one for you. I recommend it highly (and his support forums are pretty good, all things considered).

There is also a Comment Karma plugin that will do the same for user comments.

@gamerz That plugin only does a “scale” rating system (as far as I can tell), not a thumbs up / thumbs down.
