Think I broke Snippet Preview with Custom Permalinks Plugin?

So I had another plugin called Custom Permalinks that I beleive broke my snippet preview for me and haven’t been able to get it working again. I deleted the offending plugin but, still no luck.

On published posts it shows that have a post_id
-No title unless you specify one (used to default to post title)
-The Home page URL (not the correct page level URL)
-Meta description doesn’t get updated when I type in the box

Also showing me this when there is no title?
“Warning: Title display in Google is limited to a fixed width, yours is too long.”

What’s interesting is when adding a new post and before hitting save/publish everything works fine. The snippet preview updates and displays correctly.

Once a publish/post the snippet preview is acting strange and not sure if had something to do with old plugin.

Any ideas on how to fix this problem?
