theme/plugin advise

Hello everybody,

I’d like to build a websiste which allows people to enter information answering questions in order to get a final report like the one in this [link moved below]
Please when there, click on “run a test session” to see how it works.

Do you have any suggestion on what theme and plugin could do the trick.
