Florance theme – help writing php script

    I copied and pasted a form structure into a page on my site https://mysteryartcontest.com/?page_id=8

    I need help writing the PHP script. I found examples but they do not include info regarding image and video upload. I also know that there is some JS involved also. I have spent the last 12 hours trying to figure this out but everything I’m finding is done in conjunction with plugins which I’m not using. And that is due to the fact that I cannot find a plugin that allows a customizable form, image upload and video upload. Thank you
