The Send Button Has a Loading Animation All The Time & Does Not Confirm Message

I have form up at https://businessblessed.com/specialoffers/you-canceled-your-order-please-tell-use-why-so-we-can-do-better-in-the-future/ but it loads with the send button displaying an animation like it’s loading. When I do submit the form it does not display a confirmation message even though it is set to do so.
